Calling all health professionals to help share My Health, My Life, My Way with potential participants!
Individuals with chronic conditions and a disability are needed for an Online Self-Management Study. By taking part, you would help us learn whether an online chronic condition self-management program would assist in the management of chronic conditions with people with disabilities.
Participants may qualify for the study if they:
- Have been diagnosed with heart disease, chronic lung disease, arthritis, or Type 2 diabetes by a Healthcare Provider
- Converse in and read English
- Are at least 18 years or older
- Live with a permanent physical disability
- Have a smartphone or computer
This study involves:
- Access to a health coach
- Receiving access to functions with a smart home device (i.e., Amazon Alexa)
- Receiving an activity tracker device (i.e., FitBit)
- Access for disability inclusive educational content
- Participants will be compensated for their time.
