Join the first-ever, online chronic condition management program inclusive of people with disabilities!
You will be asked to participate in a 6-month study using technology to manage chronic health conditions. You will be one of two hundred (200) participants enrolled at UAB. Through randomization, you will be placed in one of four study groups. You will randomly be assigned (like a flip of a coin) by a computer.
Elements of the study will include the following:
- Access to health coaching calls for 6 months
- Access to an online telehealth platform accessible using a computer or mobile device
- Electronically delivered surveys at three timepoints (Beginning, 3 months, and 6 months)
- Access to a smart home device feature (if you own an Alexa home device like Amazon Echo)
- Access to a physical activity tracker (i.e., FitBit). With this device, we ask that you wear the device regularly. You will keep the device after the program. We will not ask you to return it.
- Access to educational content about chronic health conditions and physical disabilities.
Monetary incentives for regular participation:
For participating in the study, you can receive up to a maximum of $250. Payments will be calculated at the end of the study duration, and you will be notified by the study team about the final compensation amount you will receive.
You may not benefit directly by participating in this study. However, this will help the researchers/field to learn the feasibility of the online chronic health self-management program for future research.

The purpose of the study is to evaluate the feasibility of an online chronic condition self-management program for individuals with permanent physical disabilities.
You will be in this study for a total duration of six months. This study will include:
1) A completely online-based self-management program
2) Potential access to weekly health coaching calls to discuss self-management goals for sixty minutes per session
3) Survey packets delivered electronically at three time points over the study duration.
4) Possible phone interviews at the end of the study
5) Access to other available online self-management programs or resources
You will be compensated for your time during the study. You can receive up to $250 for participating in the study, regardless of which study group you are assigned. Payments will be calculated and distributed at the end of the study.
